Please review the following documents we require before we are able to continue the application process and determining eligibility for children ages 0-12. Further questions visit the FAQ's page

PLEASE BE ADVISED: Our mission is to

help families in need during the holiday season bring the spirit of joy to their children; however, toy distribution can be limited depending on public donations. Unfortunately, we CANNOT  guarantee that every request will be fulfilled. If you have already applied for gift assistance through another partnered organization, please do not submit another request. Multiple applications through different organizations may cause delays or removal from the list. All names of applicants will be cross-referenced with our partnered agencies to ensure toys are distributed fairly so we are able to reaooch as many eligible children within the Butte County region as possible.

Rental Agreement

Passport to Services

Proof Of Identity For kids Age(s) 0-12

Proof of Identity and Age of Children 0-12 years old for every child you are signing up is required 

For Non School Age Children Verification: Birth Certificate, WIC Card or Passport to Services

School Age Children: Birth Certificate, Passport to Services, Aeries Demographic Print Out

If your are a guardian: Proof of guardianship
